Why Balance Gets Harder as We Age and What Your Feet Have to Do with It

Most people assume that losing balance with age is just one of those things. A fact of life, like reading glasses and an increasing appreciation for a good chair. And while it’s true that balance does change as we get older, the assumption that nothing can be done about it is where a lot of people get it wrong.

Balance is a skill, and like most skills, it responds to attention and training. More importantly, the feet play a far more central role in balance than most people realise. Understanding that connection is the first step toward doing something useful about it.

This article is for older adults noticing changes in their steadiness, and for the family members who find themselves quietly watching a parent navigate a step and holding their breath.

What balance actually is and why it gets complicated

Balance isn’t a single sense. It’s the result of your brain constantly integrating information from three different systems: your vision, your vestibular system (the inner ear), and your proprioceptive system – the network of sensors in your muscles, joints, and skin that tell your brain where your body is in space.

As we age, all three of these systems gradually become less sharp. Vision deteriorates. The vestibular system becomes less sensitive. And proprioception, particularly in the feet and ankles, declines significantly. The brain is still trying to do the same job of keeping you upright, but it’s working with less reliable information from each of its sources.

The result is subtler reactions, slower corrections, and a narrower margin for error when something unexpected happens, like an uneven surface, a misjudged step, a moment of inattention.

The feet as your primary balance sensors

Your feet contain an extraordinary concentration of sensory nerve endings. Every time you take a step, the skin, joints, and muscles of the foot send a constant stream of information upward. This information includes things about surface texture, pressure distribution, angle, and movement. This is proprioception in action, and it’s happening thousands of times a day without you thinking about it.

In younger adults, this system is highly responsive. The foot detects a slight shift in balance and the body corrects almost instantaneously. With age, the sensitivity of these nerve endings decreases. Reaction times slow. The corrections still happen, but they happen a little later and a little less precisely which is often enough to make the difference between catching yourself and not.

Several things can accelerate this decline beyond normal ageing. Diabetes is one of the most significant, where peripheral neuropathy, a common complication of diabetes, directly damages the sensory nerves in the feet and can dramatically impair proprioceptive feedback. Circulation problems, previous foot injuries, and certain medications can also affect sensation in the feet.

Foot conditions that quietly undermine balance

Beyond nerve sensitivity, specific foot conditions common in older adults can significantly affect balance in ways that often go unrecognised:

Reduced foot and ankle strength

The small intrinsic muscles of the foot, and the larger muscles of the ankle and calf, play an active role in balance — making constant micro-adjustments to keep you stable. These muscles weaken with age and with reduced activity, making the whole system less responsive.

Hallux (big toe) issues

The big toe is disproportionately important for balance and propulsion. Conditions like bunions or hallux rigidus, which is stiffness or limited movement of the big toe joint, reduce the foot’s ability to push off effectively and shift the body’s balance mechanics in ways that increase fall risk.

Foot pain

This one sounds obvious but is frequently underestimated. When feet hurt, people instinctively change the way they walk. They might shorten their stride, shift weight, move more cautiously. These compensatory patterns can themselves increase fall risk by making gait less fluid and predictable. Treating foot pain isn’t just about comfort; it’s about restoring normal movement.

Thickened or painful toenails

It might seem like a minor issue, but thickened toenails pressing against the inside of a shoe can alter sensation and feedback from the toe, subtly affecting balance. Regular nail care is a more meaningful part of fall prevention than most people appreciate.

Ankle instability

Previous ankle sprains that weren’t fully rehabilitated can leave behind chronic ankle instability. This is where there is a reduced ability of the ankle to respond quickly to balance challenges. This becomes increasingly significant as the broader balance system is also declining.

Why falls matter so much and why prevention is worth taking seriously

Falls are the leading cause of injury-related hospitalisation in older Australians. Beyond the immediate physical consequences, a significant fall can trigger a cascade of effects, like reduced confidence, withdrawal from activity, loss of independence, and further deconditioning that increases the risk of future falls.

The other side of that picture is equally important: falls are not inevitable. Research consistently shows that targeted exercise, appropriate footwear, foot care, and management of contributing conditions can meaningfully reduce fall risk. This is an area where proactive attention genuinely pays off.

What actually helps

Balance and strength exercises

The proprioceptive system responds to training. Exercises that challenge balance, like standing on one leg, heel-to-toe walking, standing on an uneven surface, these stimulate the sensory pathways in the feet and ankles and improve the speed and accuracy of balance corrections over time. 

Strength training for the lower limbs supports the muscular side of the equation. Neither needs to be intense or gym-based to be effective, rather consistency matters more than difficulty.

Footwear that supports rather than compromises

Shoe choice has a direct impact on balance. Thick, soft soles reduce sensory feedback from the ground. High heels shift the centre of gravity forward and narrow the base of support. Loose or slip-on shoes that don’t secure the foot can cause the foot to move independently of the shoe on uneven ground. For older adults, footwear with a firm, relatively thin sole, secure fastenings, and a low heel is generally preferable from a balance perspective.

Bare feet or thin-soled shoes on familiar surfaces can actually improve proprioceptive feedback but should be balanced against the need for protection on hard floors and outside.

Orthotics and insoles

Custom or prefabricated orthotics can improve foot position and stability, address pain that is causing compensatory movement patterns, and in some cases improve proprioceptive input. They’re not a universal solution, but for people with specific foot alignment or pain issues contributing to balance problems, they can be a meaningful part of the picture.

Treating foot conditions that are contributing

Addressing painful nails, callus, corns, foot pain, and other conditions that affect how an older person walks and stands isn’t just routine maintenance, it’s directly relevant to fall prevention. Regular podiatry care in older adults has good evidence behind it as a component of falls reduction programmes.

A note for family members

Balance changes are often gradual and the person experiencing them may not recognise how significant the shift has been or may be reluctant to acknowledge it. Changes in gait, a new habit of holding on to walls or furniture, taking stairs more slowly or avoiding them, reducing activity, or mentioning that they feel unsteady are all worth gently raising.
